The City of Fond du Lac provides the option for employees to participate in voluntary group dental insurance through Delta Dental of Wisconsin. The employee pays 100% of the monthly premium.<\/p>\n

Delta Dental<\/strong><\/h2>\n

The City has two plans for Delta Dental benefits.\u00a0 The EPO Low plan is a narrow network and only offers in-network coverage.\u00a0 There is a $750 maximum associated with Delta Dental and a $50\/$150 deductible on the plan.<\/p>\n

The PPO High plan has a wide network with in-network and out-of-network dentists; however, out-of-network dentists will charge above and beyond Usual & Customary. There is a $1,000 maximum associated with Delta Dental and a $25\/$75 deductible on the plan. View the plan design summary and supporting documents for more information.<\/p>\n
