A completed Consent to Disclose Utility Customer Information is required for each rental property address and for each new tenant per Wis. Stat. Sec. 196.137.<\/p>\n<p><span style=\"color: #0000ff\"><a style=\"color: #0000ff\" href=\"https:\/\/www.fdl.wi.gov\/water\/wp-content\/uploads\/sites\/16\/2025\/06\/Fillable-Consent-to-Disclose-Utility-Customer-Information.pdf\" class=\"mtli_attachment mtli_pdf\">Consent to Disclose Utility Customer Information (English)<\/a><\/span><\/p>\n<p><span style=\"color: #0000ff\"><a style=\"color: #0000ff\" href=\"https:\/\/www.fdl.wi.gov\/water\/wp-content\/uploads\/sites\/16\/2023\/03\/Spanish-Fillable-Consent-to-Disclose.pdf\" class=\"mtli_attachment mtli_pdf\">Consent to Disclose Utility Customer Information (Spanish)<\/a><\/span><\/p>\n<p>Property Owners\/Management Companies may also be able to obtain\u00a0a <span style=\"text-decoration: underline\"><strong>summary notification <\/strong><\/span>for each water account, once an invoice generates from the City of Fond du Lac by submitting a completed <span style=\"color: #ff0000\">Third Party Notification for Customer Accounts<\/span>\u00a0 if owners\/property management companies comply with Wis. Stat. Sec. 66.0809(5).
