Ambulance Bill Information
- City of Fond du Lac Ambulance Bills are processed by EMS Management & Consultants, a third-party provider that specializes in medical billing. To submit insurance information or inquire about your account you may reach them at 800-827-8469. You may also provide this information online at: www.emsmc.com/patient-portal/
- If you have a delinquent unpaid ambulance bill and have received a letter indicating collection proceedings, please follow the instructions in the letter on how to proceed. You may also email: debt@fdl.wi.gov for further inquiry.
- All unpaid City receivables are turned over to the State of Wisconsin for collection.
