Filing A Claim

The following information should be submitted to allow the City to promptly act on your claim.

  1. A signed description of the incident detailing as much as possible including, date, time and place and also pictures or videos if applicable.
  2. Contact information for the claimant (address, phone number and e-mail if possible.)
  3. Proof of the amount of the claim by means of either itemized receipt or estimate.

All information should be submitted to:
