Filing A Claim
The following information should be submitted to allow the City to promptly act on your claim.
- A signed description of the incident detailing as much as possible including, date, time and place and also pictures or videos if applicable.
- Contact information for the claimant (address, phone number and e-mail if possible.)
- Proof of the amount of the claim by means of either itemized receipt or estimate.
