The City of Fond du Lac provides the option for employees to participate in voluntary supplemental benefits of Accident, Critical Illness and Hospital Indemnity through The Hartford. Supplemental benefits can help pay for out-of-pocket expenses that major medical and other insurance does not cover. The employee pays 100% of the monthly premium. The employee must submit a claim to the carrier. Once approved, the carrier will send payment directly to the employee. The employee can use the money where they need to.
Accident
Accident insurance can help you pay for out-of-pocket costs you may experience after an accident. This is a supplement to your health plan. It pays you benefits for specific injuries and events resulting from a covered accident. The wellness benefit can be claimed by each participating member on the plan once per year. Critical Illness
Critical Illness is a supplement to a health plan and pays a lump sum benefit upon a verified diagnosis. The employee & spouse premiums are calculated using the employees age. Children are covered under the employee’s premium at no additional charge. The employee has a choice of electing $10,000, $20,000, or $30,000 for coverage. Hospital Indemnity
